Home Care Center Closure Report

A home care (visiting care) center is both a long-term care institution under the Long-Term Care Insurance for the Elderly Act and an in-home elderly welfare facility under the Welfare of Older Persons Act, so closing it requires filings under both laws. Under Article 36 of the Long-Term Care Insurance Act, the closure must be reported to the competent city/county/district office at least 30 days before the planned closure date, while the abolition report for an in-home elderly welfare facility must be filed 3 months before abolition under Article 21 of the Enforcement Decree of the Welfare of Older Persons Act, so the actual schedule should be built around the 3-month deadline. The head of the institution must prepare and carry out a plan so that beneficiaries can choose and use another long-term care institution, and must settle any co-payments that need to be settled. Benefit records still within the retention period, such as contracts and service provision records, must be transferred to the National Health Insurance Service by the closure date, and a receipt is issued. If there is no replacement institution nearby and a serious disruption of benefits is expected, the city/county/district office may recommend withdrawing the closure. Hwang Yunsang Administrative Office prepares the closure report, the abolition report and the beneficiary measures plan, and files them on the client's behalf.

  • Notification Difficulty Low

You submit the statutory report form and required documents. Once the documents are in order the report is accepted; for some reports the authority checks the contents before accepting it.

  1. 01

Closure schedule: Count back from the planned closure date to the 3-month deadline for the in-home welfare facility abolition report and the 30-day deadline for the long-term care closure report, and put beneficiary transfer, record transfer and final billing on one timeline.

  2. 02

Beneficiary measures plan: Confirm the long-term care institution each beneficiary will move to, obtain that institution's acceptance and transfer date, and compile a list. Note separate linkage arrangements for beneficiaries whose guardians are hard to reach or who cannot choose an institution themselves.

  3. 03

Settlement and refund plans: Organize co-payments to be settled, unused service fees, the results of subsidy and donation use, and how remaining assets will be returned, and prepare a plan for each.

  4. 04

Filing with the city/county/district office: Submit the abolition/suspension report for elderly welfare facilities (Form No. 21) and the long-term care institution closure report (Form No. 26) with the measures plans, settlement report, and designation certificate/installation report certificate.

  5. 05

Transfer eligible care records: Compare retained care contracts and care-provision records with the transfer application, inventory and missing or damaged-record list, and transfer them to the National Health Insurance Service by closure. Original benefit claims and statements are excluded from transfer. Actual referrals and record-transfer preparation are supported only within the agreed separate scope.

  6. 06

Reporting results and wrap-up: Report the results of carrying out the measures plans to the city/county/district office by the time of abolition and complete billing for the final month. The tax office business closure report is handled separately by the applicant.

The filing-only service uses the care provider’s completed referral, settlement and record-transfer documents. Actual referral and transfer preparation are a separate option, assuming five recipients and 20 files containing up to 500 pages for inventory, missing-item and quantity checks. This is not a clinical audit of every care record. Original benefit claims and statements are excluded from the transfer.
